Responsibilities:
Your Role:
- Provide in-home physical therapy evaluations and treatments.
- Develop personalized care plans in collaboration with physicians.
- Document progress and update care plans as needed.
- Educate patients and families for better outcomes.
- Ensure high standards of care and adhere to safety protocols.
Qualifications:
What You Bring:
- Valid Physical Therapy License in NJ.
- 2-3 years of clinical experience (community-based experience preferred).
- Strong communication and computer skills.
- A valid NJ driver’s license.
Working Conditions/Physical Demand: Working indoors/outdoors, driving, lifting, sitting, standing, walking, fine motor hand movement, use of phone and tablet for documentation
