Licensed Practical Nurse – Long-Term Care - LPN1024SG#001

Shift: 3:00 PM - 11:30 PM (8 hours/day, 40 hours/week)

Job Responsibilities:

• Patient Care: Collects non-interpretable data systematically, focusing on the patient’s physiological, psychosocial, and cognitive status.
• Assistance with Daily Activities: Provides hygienic care, assists with patient ambulation and meals under the direction of a registered professional nurse.
• Patient Positioning & Care: Positions patients as needed, provides pre-operative, post-operative, and post-mortem care.
• Patient Safety: Ensures a safe patient environment by promptly answering call lights and reporting any changes in the patient's condition to the RN.
• Tailored Care Delivery: Identifies and assesses the specific needs of the patient population served and adjusts care accordingly.

Educational Requirements:

• Minimum Education: High School Diploma (HSD)
