Clinical Pharmacist

Department Overview:

Specialty services provided at Beaverton include Sports Medicine.

Function/Duties of Position:

Responsibilities may include the provision of both direct and indirect patient care, professional consultants, drug information services and other clinical pharmacy services.

The Primary Care Clinical Pharmacist duties include, but are not limited to:

• promoting the appropriate economical, safe and effective use of medication therapy
• maintaining collaborative and comprehensive medication therapy management services. This shall include physical assessment, ordering lab and diagnostic tests necessary to manage medication therapy, referring patients to appropriate health care providers as needed, performing patient triage functions and independently initiating, adjusting and discontinuing medications.
• Deliver direct care to patients through a variety of modalities which may include but are not limited to: face -to-face care, telephone, clinical video teleconferencing (when and if available), shared medical appointments, group education classes and/or secured messaging interactions.
• Document encounters and clinical decisions in electronic
• Respond to requests sent by other members of the health care team
• Maintaining a collaborative management relationship with the patient’s care team.
• Assist provider by approving refill requests as outlined by Pharmacist refill protocol (pharmacy services compliance reviewing now).
• Provide professional consultative services related to other health care providers. Professional consultative services include but are not limited drug information services (pharmacist may refer extensive, time consuming drug information request to OHSU drug information service), formulary management and medication recommendations.
• Function independently in day to day clinical and administrative duties associated with the position. In general, uses a high degree of professional judgment to solve complex problems which routinely arise throughout the work day.tions and other clinical pharmacy services in addition to traditional dispensing services and other services as required. The pharmacist will assist medical personnel and patients with drug information. the pharmacist will help provide direction to pharmacy interns, residents, and technicians. Required Qualifications:

• A current Pharmacist’s license issued by the Oregon State Board of Pharmacy.
• Community pharmacy practice.
• Drug therapy in patients described in Position Purpose. (Dosage, indications, side effects, etc.) Contemporary medication use systems (interpret prescribers’ orders, prepare and dispense medications. etc.)
• Excellent interpersonal communication skills,
• Systematic problem solving skills,ability to work independantly with minimal supervisionExcellent interpersonal communication.
• Sucessfully completes initial and continued competencies as required for maintenance of standard of practice, legal regulatory compliance and OHSU policies.

Preferred Qualifications:

• Board Certification in: Ambulatory Care Pharmacy, Pharmacotherapy, or Psychiatric Pharmacy.
• Clinic based practice.
• Oregon State Board of Pharmacy Preceptor License.
