Program Manager, Volunteer Services - Full-Time

Job Description:

Be a part of a world-class academic healthcare system, UChicago Medicine, as a Program Manager for Volutneer Services at our Hyde Park location. This position requires you to work onsite 3 days per week. You will need to be based in the greater Chicagoland area.

The Program Manager, Community Health Workers (CHWs) for the Urban Health Initiative (UHI) is responsible for managing the staff and day-to-day operations of the UHIs Community Health Worker Programs. They must be skilled in program design, development, grants, and project management.

They also are to have strong interpersonal, management and communication skills.

Essential Job Functions
  • Assuring execution of CHW programs in clinical and community settings
  • Collaborating with service line leaders and the UHI operations teams
  • Assisting with development of CHW care model designs, care pathways, etc.
  • Organizing, facilitating and overseeing CHW training
  • Managing CHW supervisor and overall CHW performance
  • Collaborating with UHI data team to ensure achievement of evaluation framework including development of data tools, data collection methods and program metrics
  • Assisting with program reporting expectations
  • Ongoing program development and implementation of best practice interventions
  • Assisting with research, learning, monitoring and evaluation tasks, and deliverables for the community benefit report
  • Developing and maintaining partnerships internally and externally that advances the mission and vision of UHI
  • Supporting the release of Requests for Proposal (RFP) process and participate in the review process to identify and support the development of local partnerships and programs that will advance the goals of the UHI
  • Developing written CHW programs protocols
  • Other duties as assigned
  • Supervises CHW Supervisor, Team Leads (19+ Directs and indirects)
  • Publications, reports, presentations and proposal contributions
  • Provide ongoing training to CHW Supervisor and CHWs to ensure quality program implementation
  • Conducts grant assessments and prepares reports necessary for program continuity and compliance
  • Provide ongoing monitoring & tracking of CHW program work plan activities and outcomes
  • Direct management of CHW program and supervisor of CHWs. Coordinates and facilitates ongoing collaborative efforts with service line lead
Required Qualifications
  • Minimum of 4 years of experience in clinical operations, community health or both
  • Exceptional interpersonal and patient service skills
  • Experience working with various Microsoft based computer software programs for Excel and Word
  • Knowledge of program evaluation and quality improvement
  • Ability to perform basic data analyses
  • Ability to solve problems independently, perform multiple tasks simultaneously, exercise good judgment, and work effectively in a team environment
  • Ability and willingness to cooperate with co-workers, supervisors, and physicians to best serve the patients.
  • Ability to learn and incorporate new systems and processes
