The Licensed Practical Nurse (LPN) is responsible for providing high-quality nursing care to patients in a skilled nursing facility. This role involves monitoring patient health, assisting with daily living activities, and ensuring the delivery of safe and effective care in collaboration with healthcare teams.

Key Responsibilities:

• Administer medications and treatments as prescribed by the physician.
• Monitor and record vital signs, patient conditions, and changes in health status.
• Assist with daily living activities, including bathing, dressing, and grooming.
• Collaborate with Registered Nurses (RNs) and healthcare team members to develop and implement individualized care plans.
• Provide education and support to patients and their families regarding health issues and care plans.
• Maintain accurate patient records and documentation in compliance with facility policies and regulatory standards.
• Ensure a safe and clean environment for patients, adhering to infection control practices.
• Participate in patient assessments, care planning, and team meetings as required.
• Adhere to all state regulations and guidelines for nursing practice.

Required Skills & Experience:

• Valid LPN license in the state of Michigan.
• Experience in a skilled nursing facility or similar healthcare setting preferred.
• Strong communication and interpersonal skills.
• Ability to work effectively in a team environment.
• Basic computer skills for documentation and reporting.

Education:

• Required:
• Completion of a Practical Nursing program from an accredited institution.

Certifications & Licensure:

• Required:
• Current Michigan LPN license.
• Basic Life Support (BLS) certification.
