Only QUALIFIED Healthcare Professionals accepted) Geriatrics - Hospital Employed Group practice is seeking an experienced Nurse Practitioner to join their team of 3 Physicians, 2 NPs (3rd NP joining early Winter), and 3 NPs who cover SNFs.

The Nurse Practitioner jointly manages patients with stable chronic illness; diagnoses and treats minor acute (episodic) illnesses; consults to initiate follow-up and monitoring and appropriate referrals for medical and non-medical conditions; and performs routine physical exams through collaboration with the physicians and within the confines of established protocol.

The Nurse Practitioner participates in the development and implementation of the clinical components. Provides primary medical management of SNF clients collaborating with physician. Conducts health assessments and evaluates patient needs at SNF.

Participates in the development of a comprehensive plan of care for each client. Participates in activities and committees related to clinical, ethical and quality care.

The ideal candidate will have direct experience working with long-term geriatric patients in either a hospital or practice setting and work Full-time. On-call consists of 1 to 2 weeknights a month and 1 weekend day|month, response to SNFs required.

Practice prefers experienced candidate. (Practitioner will rotate through 2 to 3 SNFs. The SNFs are in Lowell, MA, Billerica, MA, and Tewksbury, MA.).
