JOB TITLE: HT and Outreach Case Manager

POSITION SUMMARY

The HT and Outreach Case Manager is responsible for working with young people experiencing homelessness, victims of crime, and trafficking through our street outreach and shelter housing program. In addition, this case manager will be responsible to help manage our shelter waitlist, emergency overnight young people, and the necessary documentation to track their participation in ETO and HMIS.
CHC strives to meet the unique needs of young people we serve by providing support around housing, medical, mental health, employment and educational goals, their circles of support and overall wellbeing. Our work with young people that are trafficking survivors is very focused on being a first contact or experience/respite from the ‘life’, being willing to continually offer support, care, unconditional love in ways that support their own journey through stages of change and harm reduction.

Essential functions of the position include street outreach, intake and assessment, case planning, documentation with data entry, and basic individual and group counseling.

CLASSIFICATION: Full-Time, 40 hours per week

PAY RATE: $26 / hour

ESSENTIAL FUNCTIONS
  • Prioritize the individual needs of all young people on caseload, applying trauma-sensitive care, harm reduction and positive young people development in overall approach to case management.
  • Work hard to remove any barriers that young people that are survivors of trafficking might experience in a residential type of setting, continuing to work with them as they enter and exit the program and move through their stages of readiness.
  • Create individualized success plans in which the young people lead the goal setting process, supporting them in creating actionable ways to move themselves further along in their journey that they see for themselves.
  • Complete necessary expanded intake assessments and meet weekly minimum case management touchpoint, as well as other data systems (ETO, HMIS, TCM, CalAim) and the various requirements to be completed in a timely manner.
  • Utilize innovative approaches to engaging young people in rapport building and case management: meeting them where they are at, creating a safe, affirming space to meet 1:1, willingness to take case management outside of the building, on walks, in parks, for coffee and/or planning learning outing visits to the bank, grocery store, jobsite, etc.
  • Links young people to needed services, including making referrals, and as needed, facilitating the young people’s accessing of those services.
  • Monitor their progress and aide them in meeting their identified goals, while coaching young people to plan, implement, and follow-through.
  • Motivate young people to improve wellness along health, mental health, substance use, and other areas that will improve their overall functioning that they have expressed interest and a willingness to engage in.
  • Coordinate between other departments (clinical services, outreach, residential) and work as a team to meet young people needs.
  • Accompany the Outreach Team 2x/month to engage with Human Trafficking workers and survivors, to build rapport and connect them to services out on the street and alongside other trafficking outreach agencies, as needed.
  • Accompany the Outreach team 1x/week to engage with local young people on the street, in encampments, etc. and build rapport and connect them to services.
  • Conduct community presentations and outreach to educate and inform individuals and families that may have been impacted by all manner of human trafficking in order to promote usage of our services.
  • Work collaboratively with local and regional Anti-Human Trafficking Coalitions and its partners; communicate with appropriate community partners and collateral service providers as appropriate.
  • Attend relevant trainings and required meetings as assigned.
  • Maintain data (caseload, EO’s, waitlist, and outreach included) and complete reports as needed.
  • Attend weekly supervision with supervisor and all weekly meetings as assigned.
  • All other duties as assigned.
TRAINING, EDUCATION AND LICENSURE
  • ETO and HMIS training within 30 days of employment
  • Human Trafficking 40 hour certification within 30 days of employment
  • Crisis Prevention Institute and First Aid CPR within 60 days of employment
  • Child Protection and True Colors training within 30 days of employment

JOB REQUIREMENTS

EDUCATION: Bachelor’s Degree in Psychology, Sociology, Social Work or a related field is preferred, but not required. Will consider additional relevant experience, particularly experience performing case management for survivors of human trafficking.

EXPERIENCE: 2-3 years of professional experience working with young people experiencing homelessness or human trafficking survivors. Experience in the Human Services related fields (3-5 years) – preferred Human Trafficking, Child abuse, Domestic Violence, or Sexual Assault.

SKILLS
  • Prior Case Management Experience and an ability to build positive rapport with young people.
  • Knowledge of the full range of public and social services in Los Angeles County for low-income, homeless, and foster young people.
  • Excellent communication skills, as demonstrated by on-point, timely, and professional correspondence, both written and verbal inter-agency coordination skills (scheduling appointments and maintaining collaboration with multiple stakeholders and providers).
  • Communication with and about young people must reflect a “strength based” and “trauma informed” approach.

TRANSPORTATION Must have reliable transportation, a valid California driver’s license and good driving record.

WORKING CONDITIONS General business/office environment. Must be able to lift up to 40 lbs. on occasion, bend, sit and stand for prolonged periods of time. Some weekend/holiday work may be required.

EQUIPMENT USED Must be able to utilize basic word processing, email, and internet programs. Be able to communicate effectively orally and in written form. Computers, calculators, fax machines, telephone and other equipment required.
