Case Planner is responsibile for clients in New York Foundling’s ESSHI Harlem young adult housing program. Housing young adults age 18-25, who have aged out of care and/or are experiencing chronic homelessness. Position reports to ESSHI Program Supervisor.
Conducts home, community, and office visits; provide referrals to community service providers in accordance with the service plan for the participants; accompanies participants to drug treatment, medical, school, housing, human resource administration appointments as necessary.

Role requires comprehensive case plan assessments and delivery of appropriate interventions. The Case Planner can expect to spend at least 50% of work time in the field or working remotely and must use tools and policies to record work time and maintain communication and accountability.

Responsibilities:

Job Responsibilities:

  1. Case management responsibility for 14-18 young adults who are living in New York Foundling’s ESSHI Harlem location. May be required to report to Foundling’s Manhattan, Bronx, or Brooklyn offices.
  2. Adhere to Fair Futures Coaching model and provide Core Case Management services for each case, with the goal of supporting participants independent living skills and overall wellness.
  3. Coordinate youth’s service needs. Make appropriate referrals and follow-up with participant and service provider to ensure young adult is accessing services. Where youth is not accessing services, coach young adult and advocate on their behalf to help remove barriers.
  4. Conduct required monthly in-office and home visits with assigned cases. Complete additional meetings as needed to ensure optimal outcomes for the case.
  5. Provide flexible scheduling to complete case meetings and prioritize participants schedule.
  6. Collaborate with program vocational specialist to ensure that they are receiving appropriate vocational and educational referrals and skill building coaching.
  7. Work with young adult to facilitate their integration into the community, support their ability to live independently, support the develop of relational skills to secure a broad support network.
  8. Supervise or facilitate workshops, field trips, community days and enrichment activities for young adults in the program.
  9. Attend trainings and conferences for professional development.
  10. Maintain clear, concise and accurate documentation of progress that promote peer and supervisory review and feedback, and that demonstrates compliance with evidenced based clinical model and meeting our agency’s expectation. Assure documentation (data entry) for contacts, incidents, and reports are submitted timely (as indicated by program).
  11. Timely data entry of all services practices of the program in agency record and for reporting, and other system as provided.
  12. Receive supervision from NY Foundling leadership. Attend mandatory weekly supervision and consultation on-site at identified location by the supervisor.
  13. Has ability to maintain confidential client records.
  14. Willing to cover for team-members. Willing and able to work flexible hours. Some evenings and weekends required.
  15. Related duties as assigned by supervisor or administration to ensure that program outcomes are being reached.
  16. Be competent using smartphone, laptop, NYF identified apps and online tools (Microsoft Office) to track all work-time, task accountability, and maintain safe and responsive communication with clients and program team.

Qualifications:

Organizational Expectations include:

  • Provide case planning/case management and social work services to the pregnant and parenting youth in the maternity residence and the mother/child residential program;
  • Complete Intake interviews; psycho-social evaluation reports; collaborate and assist child care staff in developing ICMP (Individual Crisis Management Plan)
  • Collaborate with clinicians of NYF Mental Health Dept. with regard to the youth’s initial mental health screenings and annual development screenings of the babies;
  • Collaborate with the respective Hospital Pre-Natal & Obstetric Clinic staff, and NYF Medical Clinic staff regarding the on-going health care of the pregnant and parenting youth and their infants

Requirements:

  • BA/MA in Psychology, Sociology, BSW or MSW with 1-2 years child welfare experience preferred.

Other Requirements/Preferences:

  • Knowledge of community resources and ability to effectively negotiate systems for multi-problem families
  • Experience with drug treatment populations preferred
  • Bilingual Spanish/English required
  • Typing skills and computer literacy required
  • Excellent interpersonal skills
  • Demonstrated strong commitment to safety.
