Job Summary
We are seeking a compassionate and skilled Licensed Vocational Nurse for our Long-Term Care (LTC) facility. The ideal candidate will provide high-quality nursing care to residents requiring long-term support, ensuring their comfort and safety.This role requires strong clinical skills, the ability to handle diverse patient needs, and a commitment to patient education and support.
Duties- Monitor and assess the condition of residents in the LTC facility.
- Collaborate with physicians and other healthcare professionals to develop and implement comprehensive care plans.
- Administer medications, perform interventions, and manage resident care as necessary.
- Educate residents and their families about treatment processes, medications, and follow-up care.
- Maintain accurate and thorough patient records using electronic health record systems.
- Ensure compliance with infection control protocols and patient safety standards.
- Participate in quality improvement initiatives aimed at enhancing resident outcomes and workflow efficiency.
- Minimum 2 years of experience in Long-Term Care or a similar setting.
- Experience with geriatric care and chronic disease management preferred.
- Active NH State LVN license.
- BLS certification required; ACLS certification preferred.
