Job Summary

This role requires strong clinical skills, the ability to handle diverse patient needs, and a commitment to patient education and support.

• Monitor and assess the condition of residents in the LTC facility.
• Collaborate with physicians and other healthcare professionals to develop and implement comprehensive care plans.
• Administer medications, perform interventions, and manage resident care as necessary.
• Educate residents and their families about treatment processes, medications, and follow-up care.
• Maintain accurate and thorough patient records using electronic health record systems.
• Ensure compliance with infection control protocols and patient safety standards.
• Participate in quality improvement initiatives aimed at enhancing resident outcomes and workflow efficiency.

• Minimum 2 years of experience in Long-Term Care or a similar setting.
• Experience with geriatric care and chronic disease management preferred.

• Active NH State LVN license.
• BLS certification required; ACLS certification preferred.
