Details:

The social service weekend coordinator contracted or employed through Suncrest Hospice is responsible for the implementation of standards of care for medical social work services and for adherence to all conditions in the Service or Employment Agreement.

  1. The Social Service Weekend Coordinator will work closely with our social workers, chaplains, nursing leadership to ensure that patient needs are addressed on the weekends. This role requires a timely response to calls and requests, effective clinical assessment, and the ability to work independently.
  2. Assess and prioritize patient and family needs and schedule PRN and Part-time Social Workers and Chaplains for patient visits.
  3. Collaborate with weekend nursing director and team to triage urgent patient issues
  4. Speak with patients, families, senior living facilities, community agencies, to address time sensitive needs of patients
  5. May be responsible for responding to urgent patient and family needs, including but not limited to advance care planning, assisting with funeral, arrangements, urgent respite placement in skilled nursing facility, assisting with finding caregivers,
  6. Conducts evaluations, including family dynamics, caregiver abilities, communication patterns, high-risks for suicide, neglect or abuse, and plans intervention based on evaluation findings. Counsels patient and family/caregivers as needed in relationship to stress, and other identified coping difficulties. Provides crisis intervention when necessary.
  7. Conduct social work visits when indicated when PRN or On-Call staff unavailable
  8. Maintains clinical records on all patients referred to social services
  9. Provides information and referrals for patients and families/caregivers regarding practical needs and community agencies.
  10. Maintains collaborative relationships with organization personnel to support patient care.
  11. Assists physician and other team members in understanding significant psychosocial and emotional factors related to health problems and death/dying issues.

  13. Actively participates in quality assessment.
  14. Other duties as delegated by the supervisor.
The Social Service Weekend Coordinator hours are 3:00pm
  • 5:00pm Friday, 8:00am
  • 5:00pm Saturday and Sunday, and preferably On-Call Friday, Saturday and Sunday, 5:00pm
  • 8:00am. Will consider candidates able to work Saturday and Sunday only.

Qualifications:

  1. LCSW Preferred
  2. A graduate of a master’s program in social work (MSW) accredited by the Council on Social Work Education
  3. Social service leadership experience in a healthcare setting
  4. Understands hospice philosophy, and issues of death/dying.
  5. Experience in a hospice care or home health preferred.
  6. Demonstrates good verbal and written communication skills.
  7. Excellent organizational skills, ability to prioritize, work independently.
  8. Strong technical skills for EMR documentation, reporting and auditing.
  9. Must be a licensed driver with an automobile in good working condition, that is insured in accordance with state or organization requirements and is in good working order.
