Rn Clinical Manager

Key Responsibilities:

Clinical Leadership and Compliance:

• Oversee day-to-day clinical and office operations, ensuring seamless workflows and exceptional patient experiences.
• Maintain a visible presence to support staff and patients during operating hours.
• Ensure compliance with professional standards, agency policies, and state and federal regulations.
• Step in as a backup for key positions (e.g., PCM, CSS) and lead in the absence of the Territory Administrator.

Clinical Oversight of Patient Care:

• Coordinate referrals and interdisciplinary patient care to ensure a comprehensive approach.
• Oversee the development, implementation, and ongoing updates of individualized care plans.
• Evaluate clinical care quality, ensuring patient needs are thoroughly assessed and met.
• Collaborate with clinicians and physicians on patient transfers, discharges, and emergency care protocols.
• Manage daily KPIs, workflow efficiency, and escalation/complaint resolutions.

Clinical Oversight of Personnel:

• Ensure all clinical staff, including contractors, receive the required training, competency checks, and supervision.
• Assign patient and personnel tasks based on staff competencies and patient needs.
• Provide continuous coaching and mentorship to field clinicians, fostering professional growth.

Quality and Compliance:

• Lead Quality Assurance and Performance Improvement (QAPI) initiatives.
• Investigate incidents, identify trends, and implement corrective actions as necessary.
• Ensure accreditation and survey readiness through detailed QA audits and corrective action plans.
• Manage HHCAHPS distribution and oversee emergency preparedness planning and reporting.

Requirements

Qualifications:

• Bachelors degree in Nursing, Healthcare Administration, or a related field (Master's degree preferred).
• Current nursing license or relevant clinical licensure.
• Proven leadership experience in Home Health (HH) and proficiency in OASIS.
• Strong knowledge of healthcare regulations, compliance standards, and quality assurance practices.
• Excellent leadership, organizational, and communication skills.
• Analytical ability to evaluate patient care data and implement evidence-based practices.
• Proficiency in Electronic Health Record (EHR) systems and healthcare technology.
• Experience in QAPI/Survey Readiness is a plus.

Benefits

Salary: $105K - $115K
