Key Responsibilities:
Clinical Leadership and Compliance:
- Oversee day-to-day clinical and office operations, ensuring seamless workflows and exceptional patient experiences.
- Maintain a visible presence to support staff and patients during operating hours.
- Ensure compliance with professional standards, agency policies, and state and federal regulations.
- Step in as a backup for key positions (e.g., PCM, CSS) and lead in the absence of the Territory Administrator.
Clinical Oversight of Patient Care:
- Coordinate referrals and interdisciplinary patient care to ensure a comprehensive approach.
- Oversee the development, implementation, and ongoing updates of individualized care plans.
- Evaluate clinical care quality, ensuring patient needs are thoroughly assessed and met.
- Collaborate with clinicians and physicians on patient transfers, discharges, and emergency care protocols.
- Manage daily KPIs, workflow efficiency, and escalation/complaint resolutions.
Clinical Oversight of Personnel:
- Ensure all clinical staff, including contractors, receive the required training, competency checks, and supervision.
- Assign patient and personnel tasks based on staff competencies and patient needs.
- Provide continuous coaching and mentorship to field clinicians, fostering professional growth.
Quality and Compliance:
- Lead Quality Assurance and Performance Improvement (QAPI) initiatives.
- Investigate incidents, identify trends, and implement corrective actions as necessary.
- Ensure accreditation and survey readiness through detailed QA audits and corrective action plans.
- Manage HHCAHPS distribution and oversee emergency preparedness planning and reporting.
Requirements
Qualifications:
- Bachelors degree in Nursing, Healthcare Administration, or a related field (Master's degree preferred).
- Current nursing license or relevant clinical licensure.
- Proven leadership experience in Home Health (HH) and proficiency in OASIS.
- Strong knowledge of healthcare regulations, compliance standards, and quality assurance practices.
- Excellent leadership, organizational, and communication skills.
- Analytical ability to evaluate patient care data and implement evidence-based practices.
- Proficiency in Electronic Health Record (EHR) systems and healthcare technology.
- Experience in QAPI/Survey Readiness is a plus.
