Job Responsibilities:

• Maintain firm adherence to the resident’s “Bill of Rights”, as they relate to the facility
• To maintain meaningful communication with the resident’s care providers
• Assessments and coordination of admission/re-admission from the community, nursing homes and hospitals
• Coordinate and supervise clinical care for residents
• Clear and concise documentation in accordance with facility requirements

Qualifications/Requirements:

• Prior experience in Long Term Care a PLUS or 2 years’ experience as a Case Manager
• Previous case management and Compuware experience a plus
• Excellent phone skills and ability to work with families/other care providers
• Strong organizational skills, passion and compassion for the population
• Bachelor's or master's degree from an accredited college or university with major work in human resources or service delivery, and one year of full-time experience in the provision of services to a dependent adult population; or

• Ability to work cooperatively with multidisciplinary team
