Responsibilities:

As a member of the Central Assessment Team, the Inpatient LADC/Patient Intake Coordinator, housed within the Brooklyn Park hospital, collaborates with the Central Intake Team, psychiatry providers and nursing staff to integrate relevant clinical data and substantiate medical necessity for recommended level of mental health care.

This role is responsible for completing chemical dependency assessments and recommendations, as well as maintaining the referral list and follow through for current PrairieCare patients. This role is also responsible for coordinating admission to an inpatient level of care.

Responsibilities and Accountabilities:

  • Receive and review referrals from internal providers regarding chemical dependency assessments for current patients
  • Meet with current patients who screen positive for needing an assessment and complete a chemical dependency assessment, including assessment, write-up and recommendations.
  • Maintain up to date recommendation list for both referrals and recommendations for post-assessment.
  • When not performing LADC related duties, perform patient intake coordinator related duties.
  • Review, assess and link callers with information about our services
  • Triage crisis calls, assess for safety, and complete safety planning as needed
  • Facilitate internal referrals for inpatient services
  • Facilitate external referrals for inpatient services
  • Consult with physicians and nurses as needed to facilitate referrals into our inpatient services
  • Provide effective customer relations pursuant to the policies of PrairieCare
  • Provide external referral resources as needed

Qualifications:

  • Requires a Bachelor’s degree in Social Work, Sociology, Psychology or a closely related human services field
  • Licensed as a alcohol and drug counselor in the state of MN (LADC) or ADC-T licensure with plan for obtaining LADC.
  • Preferred experience of performing assessments for chemical dependency evaluations of minimum of 1 year.
  • Two years of supervised experience in behavioral healthcare is required, meeting the criteria for a mental health practitioner or a license as a Licensed Social Worker (LSW)
  • Experience in triage of psychiatric emergencies is preferred
  • Knowledge of DSM-5 criteria, possess interviewing and assessment skills and display a professional attitude in all aspects of his/her performance
  • Oriented and educated in gender, cultural differences and in age appropriate care for psychiatric and chemical dependency services
  • Must possess skills to communicate clearly and effectively with Clinicians
  • Must have computer skills using Microsoft Office Products including Word and Outlook
  • Must have reliable transportation and telephone
  • Ability to smoothly and rapidly shift between dissimilar tasks
  • Ability to communicate well both verbally and in writing
  • Ability to react quickly and calmly as part of a team to handle volatile and potentially emergency situations
  • Ability to make effective decisions while in stressful situations and circumstances
  • After training, be able to manage disruptive and explosive patients.
