Home Health Registered Nurse

Schedule: Monday - Friday, 40 hours per week; Some Call Required

Coverage Area: The peninsula, Greater San Jose including Los Gatos, Saratoga, Cupertino, Sunny Vale, and Santa Clara for day-to-day visits. On call weekends would also include Fremont down to Gilroy.

Productivity: Minimum 30 points per 40 hour week

• Perform comprehensive assessments of patients in their home environments.
• Design, implement, and modify individualized care plans in collaboration with the healthcare team.
• Administer prescribed medications and treatments appropriately and safely.
• Monitor and document patients' progress, making adjustments as necessary.
• Provide education to patients and families on health conditions and care plans.
• Facilitate communication and coordination among team members and other healthcare providers.
• Offer emotional support and guidance to patients and families throughout the care process.

• Current RN license in the state of practice.
• Experience with HomeCare Homebase Charting required.
• Strong clinical assessment skills and critical thinking capabilities.
• Excellent communication skills and the ability to build rapport with patients.
• Self-motivated with strong time management skills.
• BLS certification required.
• OASIS experience required.
• Must have at least 12 months of home health experience within the past 3 years.

• Weekly Pay Via Direct Deposit
• $2,650/wk
• Patient Related Travel Reimbursement
