American Hospice and Home Health Care is seeking dynamic and highly motivated Clinicians to join its team. We currently provide Home Health patient care in Santa Clara County and Alameda County. Full Time or Part Time available
American Hospice and Home Health Services is currently seeking a Full Time or Part Time OT to service either one or combination of these counties:
- Santa Clara County
- Alameda County
- Negotiable on salary, FT per visit, & hourly per diem pay rates depending on experience and availability. Contractors are also welcome to apply (1099)
As the Home Health Occupational Therapist, you will assess and evaluate patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments and adaptive equipment. Based on this assessment and evaluation, the Occupational Therapist helps to determine a treatment plan, performs interventions aimed at improving and enhancing the patient's wellbeing, and evaluates the patient's progress.
Responsibilities and Duties- Evaluates patient's functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, work capacity, etc.). Evaluates home environment for hazards or barriers to more independent living. Identifies equipment needs. Participates in the development of the total plan of care.
Flexible, as varied daily activities will include phone, email and written communication, meetings with a variety of healthcare professionals, great understanding of hospice rules and regulations
Must work closely with team members to provide the optimal care and services.
Ability to manage scheduling visits, documentation time, and meetings effectively.
Ability to learn an electronic medical records system.

Requirements:
Qualifications and Skills Licensed Occupational Therapist in California2 years' experience as an occupational therapist
- Current CPR Certification
- Ability to drive within 30-60 miles a day
- Ability to function autonomously
- Strong clinical skills
- Effective communication skills
- Proficient computer skills
- Reliable transportation and insurance
Compensation details: 120000-145000 Yearly Salary
