Pharmacist, Clinical – Ambulatory Care

Overview:

In collaboration with patients, families (as defined by the patient) and staff across all disciplines and departments is responsible for providing comprehensive drug therapy management under a collaborative practice agreement approved by the CommUnityCare medical staff.

The clinical pharmacist serves as an integral part of the multidisciplinary team and serves as the patient’s advocate in addressing their pharmaceutical care needs. This position will serve as a fundamental team member to continue to cultivate CommUnityCare’s team-based care model initiatives.

Responsibilities:

ESSENTIAL/PRIMARY DUTIES

  • Manage the operation of an ambulatory care clinical pharmacy service
  • Provide comprehensive medication management to ambulatory care patients as outlined in the Clinical Pharmacy protocols/collaborative practice agreement
  • Identify and resolve medication-related problems
  • Assist with medication refills as outlined in the Clinical Pharmacy protocols/collaborative practice agreement
  • Provide health improvement, wellness, and medication education to patients
  • Assume an active role in the education of students, residents, or other health care professionals
  • Serve as drug information resource for the health care teams
  • Assist with adjudication of non-formulary drug requests and act as a clinic resource regarding the formulary
  • Work within the scope of credentialing privileges
  • Collaborate and communicate with all members of the care team in providing patient-centered care
  • Empower patients and/or families to be an engaged and active participant in their care
  • Educate/train Medication Refill specialists, Clinical Pharmacist technicians’ or Clinical Pharmacist trainees when needed
  • Perform other duties as assigned
Knowledge/Skills/Abilities:
Knowledge:
  • Extensive knowledge of medical/pharmaceutical terms and nomenclature
  • High degree of knowledge of drug therapy
  • Healthcare office concepts, practices, policies, and procedures
  • Demonstrated knowledge of local, state, and federal laws pertaining to Clinical Pharmacy activities as related to the use of drugs and other substances and the delivery of care
  • Computer systems knowledge for data entry and information retrieval, including but not limited to Microsoft Office applications, and electronic health records (EHRs)
  • HIPAA and Joint Commission guidelines and regulations
  • Bilingual in English/Spanish (preferred)
Skills/abilities:
  • Excellent verbal and written communication skills, including telephone and e-mail etiquette
  • Demonstrate ability to successfully navigate and document within EHRs
  • Ability to utilize drug information resources and communicate results written/verbally
  • Demonstrate understanding and/or experience working in an integrated delivery environment
  • High level of problem-solving and critical thinking skills to independently resolve problems and knowing when to seek further clarification from supervisor, or medical providers to better serve the patient
  • Ability to work in an organized manner to prioritize tasks and complete work in a timely manner
  • Maintain strong attention to detail and accuracy

  • Complete all mandatory training and maintain appropriate credentials/licensure
  • Ensure all tasks provided and associated with patient care, patient administrative processes and related duties comply with all regulatory and accreditation standards including The Joint Commission, CommUnityCare Standard Operating Procedures and CommUnityCare Policies and Procedures

People Management/Department Management/Business Unit Management:

In addition to the Clinical Pharmacist position responsibilities and duties, team members acting in the capacity of the following positions will be responsible for additional assignments. These roles are assignments, not permanent, regular positions.

People Management

Clinic Lead:
  • Assumes full responsibility for patients receiving care within the designated health centers
  • Works in tandem with the Health Center’s practice and nursing leaders to ensure a collaborative team-oriented environment
  • Leads and motivates the medical providers under his/her supervision; uses effective medical and people performance management processes
  • Supports, leads and models the vision, mission and values of CUC
  • Serves as the champion for initiatives that impact the operations of CUC and the quality of patient care
  • Performs objective annual evaluations of all direct reports with input from the site leadership team Clinical Pharmacy Residency Program Director:
  • Assumes responsibility for activities, oversite, and coordination of the clinical pharmacy residency program.
  • Oversees the clinical pharmacy resident(s) recruitment, progression, evaluation, and documentation of residency program accreditation and completion of program requirements with input from residency program preceptors.
  • Leads and motivates the clinical pharmacy resident(s) under his/her supervision; uses effective performance management processes as defined residency program accreditation
  • Oversees the residency program preceptor’s appointment, evaluation, and development
  • Supports the continuous residency program improvement process
  • Works in tandem with the Health Center’s practice and pharmacy leaders to ensure a collaborative team-oriented environment
  • Supports, leads, and models the vision, mission, and values of CUC
  • Serves as the champion for initiatives that impact the operations of CUC and the quality of patient care as it relates to the clinical pharmacy residency program
Clinical Pharmacist Lead:
  • Assumes responsibility for activities, oversite, coordination & evaluation of the clinical pharmacy technician & medication refill specialist roles and duties
  • Oversees the clinical pharmacy & medication refill specialist technician(s) recruitment, hiring, training, competency, performance, and evaluation
  • Provides supports for clinical pharmacists’ hiring, training, competency, and performance
  • Assists the Associate Pharmacy Director with overseeing clinical pharmacy services and operations
  • Assists the Associate Pharmacy Director with the reporting, monitoring, analyzing, and evaluating quality, productivity, safety and patient health outcomes for clinical pharmacy services
  • High level of skill at building relationships, and providing excellent customer service
  • Works in tandem with other leaders to ensure a collaborative team-oriented environment
  • Serves as a representative for clinical pharmacy services within CUC multidisciplinary committees as assigned
  • Leads and motivates the clinical pharmacy team and uses effective performance management processes
  • Supports, leads and models the vision, mission and values of CUC
  • Serves as the champion for initiatives that impact the operations of CUC and the quality of patient care
  • Provides input for objective annual evaluations of clinical pharmacist team members to Associate Pharmacy Director

Qualifications:

MINIMUM EDUCATION:
  • Doctor of Pharmacy
  • One-year post-doctoral residency
  • Clinical Pharmacy Residency Program Director: Two-year post-doctoral residency in ambulatory care
PREFERRED EDUCATION:
  • Two-year post-doctoral residency
MINIMUM EXPERIENCE:
  • Clinical experience in providing drug therapy management under a collaborative practice agreement. Five years of this experience will be considered in lieu of post-doctoral residency.
  • Clinical Pharmacy Residency Program Director: Completion of PGY2 residency program in ambulatory care, followed by a minimum of three years of practice experience
PREFERRED EXPERIENCE:
  • Past preceptorship of pharmacy learners (Ex: interns, residents)
  • Basic Spanish communication skills
REQUIRED CERTIFICATIONS/LICENSURE:
  • Pharmacist license with the Texas State Board of Pharmacy
  • BLS/ACLS within 3 months of hire
  • Board certification through the Board of Pharmacy Specialties within two years of hire
  • Completion of a Postgraduate year two (PGY-1) Pharmacy Practice Residency. Five years of pharmacy experience as described above will be considered in lieu of residency.
  • Clinical Pharmacy Residency Program Director: Board certification of Ambulatory Care (BCACP) required at hire
PREFERRED CERTIFICATIONS/LICENSURE:
  • Completion of a Postgraduate year two (PGY-2) Ambulatory Care Residency REQUIRED COURSES/COMPLETIONS (e.g., CPR)
  • Basic Life Support (BLS) - Obtained through approved American Heart Association Training Network or the Military Training Network
