Position Summary: Provide Case Management leadership and management for hospitals within a market of Clients. The position will be responsible for the management or delegation of day-to-day operations, including directing and planning. Formulate and implement operational strategies and initiatives in the case management program. Ensure staff competencies in all areas of care management, including utilization review, transition/discharge planning, and social work services. Provide specialized expertise, ensure resource alignment with system-wide initiatives, and update appropriate parties on case management strategies, initiatives, outcomes, and issues. Successfully partner with physicians and others to ensure delivery of evidence-based care and reduction of avoidable clinical variation in care, Key Responsibilities: Manage and delegate day-to-day operations of the case management program.

Minimum Education: Bachelor's degree in Nursing - Required
Master's degree in Nursing, Healthcare Administration, or Business Administration - Preferred Required Skills: Working knowledge of Care Management department functions including Joint Commission and Conditions of Participation (COP) requirements.
Good understanding of Medicare, Medicaid, and other third-party payment systems.Ability to critically think, research solutions, and challenge the status quo.
Advanced computer skills including Microsoft Office; especially Word, Excel, and PowerPoint.
Strong collaborative work style and commitment to excellence in job performance.
