Position Summary:The intake associate provides an orientation to the new patient and needed information/education to new patients, so they understand how to access the full range of services at Urban Health Plan.

Minimum Qualifications:-High School Diploma required. -One year of experience in office setting with clerical duties required. Training provided on health care administrative duties.-Bilingual Spanish/English required.-Must have excellent communication skills and ability to provide exceptional experience to patients.-Understanding of phone etiquette and standard office procedures.

Key Duties:
  • Conduct intake for new patients at assigned site.
  • Complete patient assessment including screening questions.
  • Complete required new patient documentation.
  • Assure patient confidentiality.
  • Participate in quality improvement program.
