RN - Community Health Case Manager

Responsibilities:

Manage nursing care for clients living with HIV/AIDS, coordinating services and resources.
Develop and evaluate client-centered care plans, adjusting as needed to meet goals.
Provide health education on HIV treatment, prevention, and resource navigation.
Document case management activities in CAREWare and other EMR systems.
Collaborate with multidisciplinary teams to ensure comprehensive client care.
Participate in public health initiatives and emergency preparedness activities.
Prepare data reports and updates for program managers.

Qualifications:

Current Registered Nurse license.
CPR certification.
Familiarity with Epic EMR and Microsoft Office Suite.
