The Coordinated Specialty Care and Community Integration program

The Coordinated Specialty Care (CSC) and Community Integration program provides comprehensive services, including evidence-based pharmacological treatment, supported employment and education services, individual and group psychotherapy, case management, family therapy, and recovery support in the community and clinical settings.

This program services individuals ages 15-35 who have an ESMI diagnosis.

Salary: $60000 to $65000 / annually - depending upon education / experience / licensure

Hours:

This is a 40 hour full time benefitted position which includes some evenings required.

Job Summary

The Therapist will plan, organize, and implement clinical social work / counseling services to consumers and families under the clinical supervision of a qualified Supervisor. The Therapist will also plan for discharge and aftercare.

Responsibilities:

  • Maintains contact with referring agencies and related people, and with community resources used for referral
  • Maintains treatment records
  • The individual demonstrates knowledge of the principles of growth and development appropriate to the consumer population served
  • The individual must be able to assess and interpret data about the consumer’s status in order to identify each consumer’s needs and provide the appropriate care, including age-related care, needed by the consumer group
  • The individual demonstrates knowledge of the stages of change, a wellness and recovery model, and placement criteria as outlined in the American Society of Addiction Medicine (ASAM) patient placement guide

Qualifications:

  • MSW from an accredited school of social work or Master’s degree in a related counseling field with at least 1 year of experience required
  • Clinical license or license eligibility is required
  • Must be able to work under required deadlines and schedules while being flexible to special projects and requests
  • Must be able to professionally handle stressful situations
  • Must possess a valid driver’s license and if duties require driving, be able to drive an agency owned vehicle
