This role offers a short term contract employment opportunity and involves managing a caseload of formerly homeless single adults, many of whom have mental health disabilities, substance abuse issues, and/or medical issues.
As a Case Manager, you will provide comprehensive case management services with the goal of improving the life quality and housing retention of the clients.
Responsibilities:- Oversee a diverse caseload of single adults, ensuring their needs are met and progress is being made.
- Facilitate initial and ongoing assessments of the clients' needs and devise appropriate action plans.
- Proactively conduct outreach visits to ensure the well-being of the clients and address any issues promptly.
- Assist clients in maintaining their benefits and managing their rent payments effectively.
- Address any habitability issues in the clients' units and work closely with property management on routine inspections.
- Refer clients to suitable job placement programs and other essential services based on their individual needs.
- Foster a supportive tenant community by organizing regular community events and social activities.
- Maintain accurate and up-to-date documentation of client interactions and services provided.
- Respond efficiently to tenant-related crises, providing de-escalation intervention and support as needed.
- Collaborate with other departments and service providers to advocate for the clients' needs and rights.
- Ensure all communications, including phone messages, emails, and requests, are addressed in a timely manner.
- Uphold the confidentiality of client records and concerns at all times. • Possession of strong administrative assistance skills with a demonstrated ability to manage multiple tasks simultaneously.
- Proficiency in data entry, ensuring accurate and timely input of information into relevant databases.
- Experience in filing and record keeping, maintaining a well-organized and easily accessible filing system.
