Hospice Case Manager (RN)

Minimum Qualifications:

  • Must be a licensed RN in good standing with the state of Oregon
  • Possess strong computer skills
  • Have the ability to accept and adapt to different social, racial, cultural, and religious modes and fulfill the philosophy of Hospice Care

Preferred Qualifications:

  • Experience working in Hospice
  • Able to make accurate, at times, quick judgements

Responsibilities:

  • Patient visits/assessments
  • Pain/symptom – medication management
  • Teaching/education to patient/family/facility staff
