Bilingual Project Access Health Advocate

The Project Access-New Haven (PA-NH) Health Advocate reports to the Program Supervisor and works closely with the Yale University research team, patient care teams, and other PA-NH staff. In collaboration with the team, the health advocate actively engages with and coordinates the recruitment, enrollment and follow-up for participants who require assistance managing their blood pressure and enroll in PRESSURE CHECK, a multi-year study, carried out at Yale New Haven Hospital and community settings.

The study seeks to improve the quality of health care offered and to help prevent/manage hypertension outcomes following community-engaged approaches. Note: Position is coterminous with Grant Funding.

Responsibilities:

  • 1. In a professional manner, recruits, interviews patients in the community and enrolls in the study.
  • 1.1 Administers screening questionnaire in a pleasant , courteous, professional and empathetic manner.
  • 2. Communicates effectively with patients, health care providers and research staff regarding plans for intervention and referrals.
  • 2.1 All interactions with staff, research team, and community members occur in a pleasant, courteous and professional manner.
  • 3. Processes, documents, files and appropriately maintains all patients' files and forms relating to PRESSURE CHECK maintaining strict confidentiality at all times.
  • 3.1 Maintains accurate records, including screenings, interventions and referrals.
  • 4. Monitors progress and follow-up
  • 4.1 Arranges follow-up appointments with patients who have been enrolled in the study.
  • 5. Performs miscellaneous duties to assure success of PRESSURE CHECK.
  • 5.1 Recommends and promotes ideas and /or suggestions that will enhance the study.

Qualifications:

EDUCATION

Associate or Bachelor's Degree in a related field preferred, or an equivalent combination of education and experience.

EXPERIENCE

(3) years or experience with a High School Degree, (2) years of experience with an Associate Degree or (1) year of experience with a Bachelor's Degree.

Three (3) or more years of experience in one of the following:
  1. Experience working in community engaged programs and research projects.
  2. Demonstrated interests in addressing health inequity from a social determinants of health perspective.
  3. Strong teamwork skills and written and oral communications skills.
  4. Experienced with Word, Power Point, Excel, and related software.

SPECIAL SKILLS

1: Bilingual (English and Spanish) required.
2: Training as a community health worker preferred.
3: Experience addressing health inequities preferred.
  1. Ability to work flexible weekday hours as well as some evenings and weekends.

5: Ability to travel to worksites, work in-person and remotely as needed by project.

6: Ability to maintain supply inventories.
