Valeo Home Health Intake Coordinator

The Intake Coordinator is the first step in a potential Home Health patient’s experience. The intake coordinator will receive the referral and deliver it for review. Once reviewed, the intake coordinator will notify the referral source of receipt and request any additional information as needed.The patient will be entered into the charting system, eligibility will be verified, and Start of Care Outcome and Assessment Information Set (OASIS) and Initial Evaluations will be scheduled for all disciplines ordered. The intake coordinator will complete any follow up communication with staff and/or referral source as needed.
Responsibilities:
- Performs intake coordinator duties as assigned for one or more branches.
- Reviews referral method and source and passes it on for review.
- Enters patients into the charting system and verifies eligibility correctly.
- Verifies visits are completed in a timely manner and appropriate documentation is completed within regulatory timeframes.
- Works to facilitate timely admissions.
- Demonstrates knowledge of regulatory guidelines for visit timeframes and documentation requirements.
- Understands the regulations and requirements for admission.
- Performs job in compliance with agency policies and procedures as well as community and professional standards.
- Communicates necessary information to the supervisor and management team to ensure coordination of services and activities.
- Completes assigned training on time.
- Performs other duties as assigned.
Qualifications:
- High School or College Education.
- Business and professional setting experience.
- Health care experience, preferred.
- Excellent written and verbal communication skills.
- Speak, read, write and comprehend English, other languages preferred.
- Ability to handle multiple tasks simultaneously and meet deadlines.
- Excellent organizational and time management skills.
- Self-starter.
