Greenlife Healthcare Staffing is currently seeking a Licensed Practical Nurse to fill an opening within an Assisted Living Facility which is consistently rated five stars by governmental agencies located in Brooklyn, New York. This facility has consistently received top ratings from governmental agencies, reflecting its commitment to providing exceptional patient care.
Requirements of a Licensed Practical Nurse:- Must have at least 6 months of clinical experience.
- Must have an active New York state license
- This is a Part-time position.
- Sunday, Tuesday, & Wednesday (7am - 5pm).
- The salary for this position is $35 | hr.
The Adult Care Facility Nurse, LPN will perform tasks and responsibilities within the framework of case finding, health teaching, health counseling, and provision of supportive and restorative care under the direction of a registered professional nurse.
Responsibilities of Licensed Practical Nurse:- Call the home care coordinator, RN, or supervisor with a verbal report of initial findings.
- In conjunction with the family and physician and|or authorized practitioner, the American Supervisor adheres to the Plan of Care based on the RNs assessment.
- Provides nursing care as outlined in the Plan of Treatment to include the following:
- Nursing services, treatments, and preventive procedures, including those requiring substantial specialized skill and ordered by the physician or authorized practitioner.
- Initiates preventive and rehabilitative nursing procedures as appropriate for the patients care and safety.
- Evaluates the effectiveness of his|her nursing service to the individual and family.
- Observes signs and symptoms and changes in the patients physical or emotional condition and consults with the physician or authorized practitioner concerning alteration of the plan of treatment in consultation with the supervisor, as necessary.
- Provides and completes nutritional assessments under the direction of the Director of Nutrition.
- Reports all patient changes and information to the Primary RN, Supervisor, or Case Manager of home care on a timely basis.
- Submits timely records of visits and vouchers based on each visit.
