OUR MISSION:

Family Reach is a national 501(c)(3) nonprofit dedicated to removing the financial barriers standing between cancer patients and their treatment — such as loss of income, increased out-of-pocket expenses, and socioeconomic factors. When faced with these financial barriers, families have to make impossible decisions like choosing between paying for treatment or basic essentials like housing, food, and transportation.

No family should have to choose between their health and their home — and we won’t stop until that’s the reality.

POSITION OVERVIEW:

From the moment of diagnosis, race, ethnicity, income level, zip code, and language create financial and clinical barriers to cancer care. These factors should not define a patient’s chance of survival, but they do.

With a focus on supporting patients with their food, transportation, housing and utilities, Family Reach supports meets patients' basic needs so that they can focus their attention on their or their loved one’s treatment.

The Resource Navigator will be responsible for the delivery of the Resource Navigation program to people facing cancer. They will manage a caseload of patients and caregivers who require additional support in navigating the landscape of assistance programs to help afford their basic needs through cancer treatment.

The navigator will need to work effectively with populations who are disproportionately affected by the financial burden of cancer, prioritizing their most critical needs. They will need to have a deep knowledge of the social service and health insurance landscapes, such as benefits coverage and eligibility for government programs.

Due to the high stress nature of cancer treatment, they will need to be confident in skills such as crisis management and conflict resolution, and effective time management to ensure prompt service.

The ideal candidate will have a balanced skill set including social work assessment, rapport building, trauma informed care, oncology, and a passion for equity in healthcare.

In order to work with our diverse client population, Family Reach is seeking a candidate who is bilingual in English and Spanish. This position is for a Licensed Social Worker. License requirements as set by state working from; MSW preferred. This is not a clinical position.

KEY RESPONSIBILITIES:

Resource Navigation
  • Work directly, in a remote environment, with people diagnosed with cancer, their caregivers, healthcare professionals (HCPs), or community advocates to provide navigation to services
  • Operate with compassion, respect, and empathy for each patient and their unique needs
  • Assess patient needs in the areas of affording food, transportation, housing and utilities
  • Identify resources the patient will be eligible for that meet the identified needs
  • Facilitate the connection of patients and/or caregivers with resources to meet identified needs
  • Manage a caseload of applicants using electronic documentation and record keeping, ensuring a high level of data accuracy and organization
Database Administration
  • Update internal database with programs that support basic needs to improve our knowledge of the resource landscape
  • Update referrals statuses and track outcomes in order to demonstrate program effectiveness
  • Manage database administrative tasks to support the Cancer Equity Initiatives team and cross-departmental stakeholders
Cross-Functional Collaboration
  • Share information and success stories, challenges, and insights about how to improve Resource Navigation
  • Actively participate in weekly team meetings to share ideas, suggest improvements, and learn from other team members
  • Work across departments to identify families to represent the Family Reach mission for various initiatives, events, and materials
  • Assist with special projects as needed

QUALIFICATIONS:

  • 2-4 years of professional work experience, especially in oncology
  • Licensure in Social Work- license requirements as set by state working from; MSW preferred
  • Bilingual - Spanish speaking
  • Excellent oral and written communication skills
  • Proficiency in technology suited for remote work
  • Highly organized and detail-oriented
  • Ability to be both collaborative and independent
  • Solutions-oriented; eager for ongoing learning and self-improvement in the role

COMPENSATION

The salary range for this position is $50,000-$60,000. Family Reach offers an attractive benefits package, including, but not limited to, an annual performance bonus, medical, dental, vision, retirement plan, and generous vacation and leave policies.
