Overview:

Under direct supervision, the Behavioral Health Navigator will address behavioral health needs through improving access to services and serve as a resource for the community at large within the defined service areas of wellness and recovery.

The role of the Navigator does not include the provision of direct services – the focus will be on supporting the seamless access to behavioral health services. The person in this role will understand challenges related to behavioral health, understand how to connect people with resources, and how to access services that are available.

The Navigator is expected to become fully knowledgeable with the behavioral health continuum of care, as well as all providers and advocacy agencies that serve Middlesex County residents.

  • Screen incoming inquiries through phone, email, portal, or direct referral.
  • Assess the needs of the individual or family to determine an appropriate course of action.
  • Refer individuals/families to clinical, social, or other support services, based on needs assessment.
  • Educate and empower individuals/families.
  • Prepare detailed reports and records; maintains files and confidentiality.
  • Establishes and maintains external relationships with agencies, non-profits, etc.
  • Maintains log of referrals; documents and updates, as needed.
  • Other duties and projects as required by Middlesex County Office of Human Services

Qualifications:

Qualifications:

Education: Degree in Social Work, Public Health, Human Services, Psychology or other related major preferred, but not required.

Experience: Related experience working on non-profit, behavioral health, government, or related field desired. Lived experience related to this role is also valued!

Languages: Bilingual (Spanish) required
