Position Summary:
Provides direct and indirect social work services to assigned resident care units including pre-hospital services, in-hospital service and discharge planning services. Assist residents’ and their families in seeking help with emotional, social, psychological and financial problems.

Responsibilities:
- Conducts discharge planning meetings on assigned floors together with nursing and support services in order to locate residents’ in need of social work services/discharge planning.
- Interviews residents’ and/or significant others to obtain pertinent background/social/financial data. Assists resident/family members in locating appropriate community services to meet their specific needs.
- Interprets available programs and services to patient, family and residents’ physician. Refers patients to social agencies to provide services required.
- Uses casework method in counseling with resident and family as needed.
- Cooperates with hospital staff and physicians and keeps them informed of progress with residents via verbal communication and chart notes in residents’ medical record.
- Maintains monthly statistical report on status of caseload.
- Assists in maintaining a safe environment for all residents which may include reporting to appropriate managerial staff equipment that is out of order, suspicion of resident abuse, mistreatment, neglect and reporting of suspicion of criminal activities.
- Follows policies, procedures, and safety standards. Completes required education assignments annually. Works toward achieving goals and objectives, and participates in quality improvement initiatives as requested.
- Performs other duties as assigned.
Qualifications:
Minimum Education:
Bachelor’s Degree in social work or a bachelor’s degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology. Master's Degree preferred.
Minimum Experience:
One (1) year of supervised social work experience in a health care setting working directly with individuals.
