PROGRAM COORDINATOR - Day Habilitation Program

POSITION(s) SUPERVISED: N/A
EDUCATION/EXPERIENCE REQUIREMENTS: Bachelors degree in related field and at least 2 years of direct industry experience. The 2 years required experience working in this or a relevant field can be substituted for a bachelor’s degree.
OBJECTIVE: To ensure the highest quality of services to the people in our day services. All program participants will have a clean and safe environment in which to work while participating in an active, creative and supportive programming. All staff will be trained and supervised to provide services that place the individuals first.
ESSENTIAL SKILLS: Must be able to successfully relate, teach and further develop skills with the people served, engage positively with members, staff and other stakeholders; respect and implement the NUPATH Mission and values. The person in this position must be able to verbally communicate in a polite, courteous and professional manner at all times, read and write in English, monitor the safety of all individuals and verbally present updates on individual status.Must adhere to all NuPath policy and procedures. Must obtain MAP certification within 6 months of employment.
PHYSICAL DEMANDS: Must have the physical ability to stand frequently during the day, engage in repeated stooping or bending at the waist and be able to go up and down stairs several times during the shift. Must be able to safely support, pivot, physically move or assist a person weighing up to 200 lbs.during Activities of Daily Living, with or without assistance. Must be able to physically ensure the safety of all individuals in the program by safely participating in established behavioral intervention.
Responsibilities:
- Coordinates and oversees all internal program staffing on a daily basis by the development and implementation of schedules.
- Assists the Program Manager in hiring, training and supervising work-study and other student interns.
- Trains and onboards staff as assigned.
- Participates in direct support coverage as needed and scheduled
- Must maintain all required certifications
- Must attend all required trainings and meetings
- Provides advocacy and case management services for clients, which includes contact with families, residences and other service providers.
- Implements and oversees consistency with clinical and behavioral treatment plans.
- Provides crisis assessment, prevention and needed intervention.
- Coordinates all program activities.
- Completes and maintains attendance both in program and ASD house groups.
- Provides specialized training and instruction to staff and individuals.
- Assists the Program Manager in the development and initiation of program goals.
- Resolves problems, runs meetings and assigns tasks as requested
- Attends weekly staff and supervision meetings as required by the Program Manager and/or Director.
- Maintains and oversees accurate program documentation such as incident reports, daily logs, progress notes, teaching programs, and any other documentation as requested
- Completion/submittal of Incident Reports
- Maintain First Aid, CPR, MAP and PREVENT certification
OTHER DUTIES:
- May be asked to drive occasionally
- Abides by the NuPath Dress Code
- And other duties as assigned by supervisors
- MUST HAVE A VALID DRIVER’S LICENSE
Compensation details: 23-25 Hourly Wage
