In response to the humanitarian crisis at America’s ports of entry, Devereux provides short-term shelter care services for unaccompanied children who have come to the United States without a parent or legal guardian. Devereux provides extremely high-quality, trauma-informed residential and educational services for unaccompanied children, with support for their social and emotional health.We also ensure they receive excellent and immediate physical healthcare including pediatric, dental and nutritional supports for every child.
Devereux’s ultimate goal is to ensure every unaccompanied child who receives our services is successfully and quickly discharged into the loving care of their family sponsor. During the short time they are with us, we ensure they have received best-in-class medical, therapeutic, recreational and educational services designed specifically to address the traumas they have sustained during, and often before, their migration journey.This is a very high benchmark to set, but we have already begun partnering with leading legal defense nonprofits to ensure children in our care have access to the finest immigration advocates and support our country offers.
Schedule:
Monday to Friday 8:00am to 5:00pm. (No Remote)
Position:
The Bilingual Case Manager is responsible for case-specific advocacy and case monitoring activities, ensuring the safe and expedited release of children to potential sponsors that meet the Short-term Shelter program criteria for release. The Case Manager, in consultation with clinician(s), is responsible for assessing the needs of each child in care and develops, implements and coordinates the Individual Service Plan (ISP) for each child.The Case Manager reports to the Lead Case Manager. The Case Manager has a caseload of 8 cases.
Primary Responsibilities Include:
- Develops master treatment plan for child, family and all necessary providers, monitors plan to ensure progress and compliance
- Attend medication appointments or psychiatric evaluations as needed
- Serves as the liaison for resource and is responsible for ensuring all referral are made for medically necessary treatment
- Initiates, develops and maintains collaborative relationships with children, their family members and significant support systems
- Facilitates meetings and has primary responsibility for completion and compilation of documentation from all internal and external providers
- Serves as liaison on the team for system navigation in obtaining community services and entitlements, securing appointments with outside services and agencies as needed, including identification of funding sources and linkages to other agencies/services
- Complete referrals, assessments, treatment plans and required documents
- Demonstrates knowledge and skill snecessary to obtain appropiate care, treatment and services for the behavioral health needs of the children
- Serves as primary liaison in regard to all levels of care, discharge planning and/or any other area that the family needs support

Qualifications:
- Bachelor’s degree (behavioral science, human service, or social service field preferred)
- Have the ability to work in conjunction with clinicians and other team members
- Be bilingual in English/Spanish
- Must be empathetic and have a team approach to care for our individuals.
- Must have excellent written and verbal communication skills.
Additionally:
- Fully immunized/vaccinated (or have a medical/religious exemption) for Varicella, MMR, TdAp, Influenza prior to starting.
- Must be 21 years of age or older (per licensing standards).
- Valid Connecticut Driver's License.
