Clinical Pharmacist Infusion Center

Responsibilities:
- The Infusion Center Clinical Pharmacist will have specialized knowledge about pharmaceuticals and their use in treating cancer and its comorbidities.
- Pharmacists on the oncology health care team work collaboratively to optimize outcomes by ensuring that the correct drug is used in the most appropriate dose for individual patients and in adherence to the correct treatment dosing schedule.
- These pharmacists are more frequently included as part of the clinical oncology team, conferring with the other team members, and contributing key drug information in the development of patient-specific and often innovative care plans.
- The pharmacist will also have specialized knowledge of the non-cancer infusion therapies from a variety of medical specialties that are administered within the Infusion Center.
- The pharmacist accepts responsibility, authority, and accountability for management and provision of care in accordance with the current policies and procedures.
- The pharmacist is responsible for leading efforts in the prevention and reporting of medication errors and adverse drug reactions according to policy and works to continuously improve process and practice in the institution to enhance safety of medication therapy.
- This pharmacist must maintain competence in all aspects of current clean room operations, aseptic techniques, and pharmacy standards including the safe handling of hazardous medications according to USP chapters , .

Education Requirements- Doctorate Degree - OR - combination of relevant education & experience may be considered in lieu of Doctorate Degree
