Job Summary:
- The Licensed Practical Nurse (LPN) provides high-quality, compassionate care to residents in a skilled nursing facility. This role is crucial in supporting the healthcare team in delivering individualized patient care, ensuring that residents receive the highest standards of nursing practice. The LPN will work closely with Registered Nurses (RNs), physicians, and other healthcare professionals to promote optimal health outcomes for residents.
Responsibilities:
- Administer medications and treatments as prescribed by physicians and as outlined in care plans.
- Monitor and document patients' vital signs, health status, and changes in condition.
- Assist in the development and implementation of individual care plans for residents.
- Provide direct patient care, including bathing, dressing, grooming, and feeding residents.
- Communicate effectively with residents, families, and the healthcare team regarding patient care and concerns.
- Ensure compliance with all health and safety regulations, as well as facility policies and procedures.
- Participate in resident assessments and contribute to care planning and evaluation.
- Assist with procedures and care provided by RNs and other healthcare professionals.
- Maintain accurate and timely documentation of all patient care activities.
- Provide education and support to residents and their families regarding health management and care needs.
Qualifications:
Education:
- Graduate of an accredited LPN program.
Licensure/Certification:
- Current New York State LPN license
- Basic Life Support (BLS) certification.
