Job Title: Patient Navigator - Healthcare Access Specialist

Duties:

  • Facilitate patient access to healthcare services by scheduling appointments and coordinating care pathways.
  • Serve as a patient advocate, addressing concerns and providing resource guidance.
  • Maintain detailed and accurate records using Epic and Excel.
  • Communicate with patients face-to-face to ensure understanding of treatment plans.
  • Collaborate with social work and healthcare teams to address patient needs comprehensively.

Skills Required:

  • Strong communication skills and ability to use Epic effectively.
  • Experience in a patient-facing healthcare role.

Education:

  • Associate degree required.

Pay Range:

  • Weekly: $1,275 - $1,525
