Job Title: Patient Navigator - Healthcare Access Specialist
Duties:
- Facilitate patient access to healthcare services by scheduling appointments and coordinating care pathways.
- Serve as a patient advocate, addressing concerns and providing resource guidance.
- Maintain detailed and accurate records using Epic and Excel.
- Communicate with patients face-to-face to ensure understanding of treatment plans.
- Collaborate with social work and healthcare teams to address patient needs comprehensively.
Skills Required:
- Strong communication skills and ability to use Epic effectively.
- Experience in a patient-facing healthcare role.
Education:
- Associate degree required.
Pay Range:
- Weekly: $1,275 - $1,525
