Staff Pharmacist, Inpatient Infusion - Full-Time, Days

Job Description:

Be a part of a world-class academic healthcare system, Ingalls Memorial, as a Staff Pharmacist-Infusion in the Inpatient Pharmacy.

In this role as an Infusion Pharmacist, you will provide a quality level of comprehensive pharmaceutical services demonstrating sound judgment based upon knowledge and application of professional, departmental, and organizational principles and standards.

The person in this role demonstrates the necessary skills and knowledge to provide care to the patient population served by the organization that includes pediatric, adolescent, adult and geriatric patients. The Infusion Pharmacist is responsible for providing pharmaceutical care consistent with departmental/hospital philosophy, mission statement, professional standards, and policies and procedures, in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.

The Pharmacist Observes and adheres to all corporate compliance requirements and communicates issues as they arise to the appropriate parties as identified in the Corporate Compliance Plan.

Essential Job Functions
  • Demonstrates an understanding of the department policies and procedures concerning infusion operations.
  • Maintains contemporary knowledge of state and federal pharmacy laws and regulations
  • Demonstrates ability to operate essential equipment & programs in the department.
  • Practices in accordance with current state and federal pharmacy laws and regulations
  • Accepts and understands full responsibilities of work assignments
  • Works independently with minimal supervision.
  • Exhibits timely, satisfactory completion of assignments with minimal assistance, as evidenced by no overtime unless approved by management team.
  • Demonstrates a commitment to enhancing the services of the department through personal actions and/or participation on departmental ad hoc performance improvement teams.
  • Reports cost saving (expense reduction) possibilities to pharmacy supervisory personnel for immediate consideration.
  • Demonstrates actions that contribute to meeting or exceeding the department’s financial goals.
  • Monitors inventory usage of all drugs and pharmaceutical items throughout the pharmacy and requests replenishments as required.
  • Sustains the formulary by minimizing non-formulary procurements, utilizing therapeutic substitution protocols, and promoting rational medication therapy selection, and suggesting appropriate Formulary alternatives to the physician as indicated
  • Ensures the maintenance and proper operation of equipment and supplies in the work areas in accordance with all USP 797 and 800 requirements.
  • Demonstrates the ability to implement problem solving mechanisms in routine and emergency situations
  • Documents or takes corrective action regarding real or potential personnel problems and refers same to appropriate supervisory personnel for follow up and resolution.
  • Demonstrates ability to identify and resolve interpersonal conflicts constructively when dealing with members of the health care team at all times.
  • Maintains confidential patient and departmental information
  • Reports any interpersonal conflicts, which requires intervention to supervisory personnel for assessment of perceived problems.
  • Utilizes appropriate hospital chain of command in resolving complaints, grievances or problems with no incidents of inappropriate utilization noted annually.
  • Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic, religious/spiritual needs of patients and their families.
  • Assures that proper labeling practices as required by State board of Pharmacy.
  • Ensures that technicians follow proper aseptic technique in preparing IV admixtures
  • Utilizes Dose Edge IV Workflow processes for preparation and check of completed admixture for accuracy and clarity.
  • Oversees chemotherapy medications preparation and adherence to hazardous material waste guidelines in accordance to our approved therapy plans and Epic build.
  • Follows and adheres to all the P&P set by USP 797 and 800 in all aspects of order entry, verification, and compounding as well as labeling and dispensing functions.
  • Acts as the Pharmacy lead and point person to ensure all clinical engineering and monitoring requirements met and kept in accordance with USP 797 and 800.
Required Qualifications
  • The incumbent must remain alert on all occasions, and must be able to effectively intervene if questions arise as to the appropriateness of a medication order
  • The ability to significantly affect patient care is constant
  • Minimum of 2-4 years of Infusion Pharmacy experience
  • The incumbent gives work direction to other department personnel
  • Basic Life Support certification.
  • Licensed in State of IL and in good standing
  • Must hold current valid registration as a pharmacist in the State of Illinois
  • Attained the professional degree in pharmacy from an ACPE accredited school (Pharm.D.)
Preferred Qualifications
  • 2 years or more of hospital experience as an infusion pharmacist
  • PGY-2 in Ambulatory desired
  • Prior experience with Epic and Dose Edge systems are highly desired
