Join UChicago Medicine as a Home Health Nurse Liaison (Hospice)

The Home Health Nurse Liaison meets with patients and families to facilitate the home health admission process.

Essential Job Functions:

  • Evaluates referrals for home health needs and medical necessity
  • Provides a complete physical assessment and determines if patient meets criteria for hospice
  • Completes an initial assessment of patients to determine home health care needs
  • Assesses patients to ensure they meet home health criteria for admission
  • Meets with patients and families to discuss goals of care
  • Educates patients and families on home health services
  • Coordinates with the referring sources and facilitates the home health admission
  • Coordinates with hospital case management to facilitate patient admissions to home health
  • Performs initial communication with the physician regarding the patient’s needs.
  • Provides direct patient care as needed.
  • Documents the initial patient and family assessments and interventions.
  • Prepares clinical notes and updates the primary physician when necessary.
  • Ensures that arrangements for equipment, medication and other necessary items and services are available.
  • Collaborates with the Intake Departments and clinical staff to manage referrals/admissions.
  • Customer/Referral Source Management: Primarily works with hospitals and other potential referral sources to facilitates admissions
  • Works closely with clinical staff to ensure services are provided timely and efficiently
  • Other duties as assigned.

Position Qualifications:

  • Graduate of an accredited school of nursing.
  • Current state of Illinois nursing license as an RN.
  • Possess excellent collaboration, observation, verbal and written communication, and problem-solving skills.
  • Current CPR certification required.
  • One to two years of recent acute care experience in an institutional setting.
  • Previous Hospice or Home Care experience preferred.
  • Nursing skills per competency checklist.
  • Possesses and maintains a valid driver’s license and current automobile insurance and has personal and dependable mode(s) of transportation to conduct home visits as necessary.

Physical Requirements:

  • No heavy lifting and nearly all work is performed in a comfortable, indoor office facility.
  • Job frequently requires sitting and handling objects with hands.
  • Fine motor control/precision is required for data entry tasks.
  • Job requires standing, walking, reaching, talking, hearing, and lifting up to 10 pounds.
  • This position requires frequent travel to locations on and off campus, driving is required.
  • Vision requirements:
  • Ability to see information in print and/or electronically.

Continuing Education Requirements:

UCM Home Care personnel are expected to participate in appropriate continuing education as may be requested and/or required by their immediate supervisor. In addition, personnel are expected to accept personal responsibility for other educational activities to enhance job-related skills and abilities and as required by licensing standards.
