Seeking for a Full-time Home Health LVN in the San Antonio, TX and surrounding areas.

Shift/Schedule: Mon-Fri from 8-5 p.m. with 1 on call weekend per month.

Responsibilities

Skilled Visits
Assists in development, review and revision of the plan of care.
Provides quality patient care set forth by the RN Case Manager's Care plan
Observes, records and reports reaction to treatment and any changes in client’s condition to appropriate personnel and/or physician.
Provides ongoing instruction and supervision of client, family or other health team personnel in the plan of treatment.

Requirements:

Geriatric Home Health Experience
Texas Nursing License
Strong organizational and self-management skills
Strong and compassionate customer service skills
Valid driver’s license
