Organization Description:
The International Institute of Minnesota (IIMN) is a dynamic organization that provides services to maximize the talents and opportunities New Americans bring to our state. IIMN assists low-income New Americans in their transition to a new homeland by providing comprehensive services for successful integration.
IIMN serves more than 4,000 New Americans annually with new arrival social services to employment placement and advancement through to U.S. Citizenship attainment. Programs are divided into three primary areas: Refugee Services, Education and Employment Training Services, and Immigration Services.
IIMN also provides advocacy for policy reforms and supports systems change that improve opportunities for New Americans to move their lives forward.
Purpose:
The Medical Case Manager is an essential member of the Refugee Services team who supports the Reception & Placement Program (R&P) program, which provides case management services to newly arrived refugees for up to their first 90 days in Minnesota.
In this position, the Medical Case Manager will deliver core R&P services to his/her/their own caseload, including greeting arrivals at the airport; providing transportation to necessary appointments such as the Social Security office, health clinics, county offices, and schools; connecting adults to employment services; educating families on budgeting strategies; and promoting independence and self-sufficiency throughout the case management period.

The Medical Case Manager also orients clients to their new community and supports them in navigating and accessing available community resources. The Medical Case Manager will primarily work with medically complex cases requiring significant healthcare system navigation support for serious acute and/or chronic conditions, by assisting in identifying barriers to independence and creating strategies for overcoming those challenges.

Position Duties and Responsibilities:
  • Read and understand program requirements of the R&P program
  • Meet refugee families at the airport and transport them to their new home
  • Conduct home safety visits and cultural orientations to life in the United States
  • Complete intake paperwork and assist clients in creating short- and long-term goals
  • Complete all required home visits and paperwork in a timely and efficient manner
  • Assist with applications for public benefits such as SNAP, cash assistance, health insurance, SSI, WIC, childcare, disability, and other community resources
  • Arrange appointments for public assistance, medical visits, Social Security, school enrollment, and other necessary appointments within required timeframes
Teach adults how to navigate public transportation and ensure independence in utilizing transportation systems in the Metro Area
Complete cultural orientation on such topics as housing, budgeting, healthcare, transportation, and employment services
  • Interpret or arrange for interpreters for clients and/or family members as necessary
  • Make referrals to supplemental services that will help advance clients’ goals
  • Document tasks in case notes in an organized and timely manner and complete necessary and required paperwork for each case
  • Attend weekly and monthly, and staff meetings
  • Other duties as assigned
Requirements:
  • Bachelor’s degree in healthcare, public health, or social work; or 2 years of work experience
  • Must be capable of working in a fast-paced, multicultural office environment, take initiative, and be able to handle and prioritize multiple assignments at once
  • Commitment to the use of strengths-based client approaches, and to supporting families in their journeys to becoming confident, independent, and self-sufficient
  • Ability to speak Arabic, Somali, Burmese, French, Karen, Spanish, or Swahili preferred
  • Commitment to the human rights and general welfare of asylum seekers, refugees, and immigrants
  • Valid driver’s license and access to own transportation required; must be willing to transport clients in personal vehicle
  • Ability to work some late nights and weekends
  • Strong organizational skills and attention to detail including comprehensive case noting
  • Skilled in problem solving and able to independently address issues as they arise
  • Experience with Microsoft Office applications, including Word, Excel, and Teams
