Lutheran Haven is seeking a caring, responsible Home Health Aide to provide personal care services to patients in the home setting. All services provided by the Home Health Aide are under the supervision of a registered nurse, Physical Therapist, or Speech Therapist.
Job Duties:
- Actively supports Lutheran Haven’s Mission and Ministry.
- Demonstrates a willingness to be an effective team member by fostering positive relationships with teams and residents.
- Provides direct and indirect personal care to the patient in the home setting under direction of a registered nurse or therapist.
- Utilizes safety measures in provision of care.
- Maintains effective communication with supervisor regarding changes in paatient's behavior or condition.
- Documents and submits accurate accounts of services provided.
- Prepares patient's medication and accompanying necessary items, i.e. juice, water, cups, spoons, etc.
- Reading and recording temperature, pulse and respiration.
- Participates in case conferences.
- Assures that established infection control and universal precautions are maintained when performing duties.
- Attends mandatory in-services.
- Other duties as assigned.
Minimum Qualifications:
- Must have a current certification in the State of Florida as a nursing assistant or show proof of application for reciprocity from another state, or proof of application for test seat.
- Successful completion of at least 75 hours of classroom and practical training with 16 hours of supervised practical training. (Federal)
- High School diploma or equivalent preferred.
- Ability to demonstrate sound judgment and ability to function on a professional level with clients, staff, and other outside professionals during stressful or emotional situations.
