Responsibilities- Conduct comprehensive assessments of clients' needs and develop individualized case plans.
- Provide ongoing support and follow-up to clients throughout their service journey.
- Facilitate access to essential services such as legal aid, housing, and counseling.
- Monitor client progress and adjust case plans as necessary to ensure successful outcomes.
- Collaborate with community organizations to strengthen service referrals and partnerships.
- Document all client interactions and maintain up-to-date case files in compliance with organizational policies.
- Participate in training and professional development opportunities to enhance case management skills.
- Bachelor's degree in social work, psychology, or a related field preferred.
- Minimum of 2 years' experience in case management or a related social services role.
- Strong knowledge of resources available for women's rights advocacy and support services.
- Excellent communication and interpersonal skills, with a compassionate and empathetic approach.
- Ability to work collaboratively in a team-oriented environment while also being self-directed.
- Strong organizational skills and the ability to manage a diverse caseload.
- Proficiency in relevant computer applications and case management software.
