Job Title: Social Work Coordinator - LMSW

Duties:

Conduct psychosocial evaluations and develop patient care plans.
Support patient transitions from hospital to home or other care facilities.
Manage documentation using EPIC, ACM, and Careport systems.
Provide guidance and counseling to patients and families in crisis.
Work collaboratively with interdisciplinary teams to optimize patient outcomes.

Skills Required:

Hospital-based social work experience with EPIC and ACM systems.
Proficiency in discharge planning and emergency response.

Education:

Master’s Degree in Social Work and a valid LMSW license.
