Clinical Liaison

The Clinical Liaison is responsible for maintaining collaborative relationships between LSRVNA and its referral sources, hospitals, facilities and provider offices. The Clinical Liaison works closely with referral sources and LSRVNA intake department to ensure seamless transition of patients into our care.The Clinical Liaison presents education to the community, referral sources, skilled nursing and assisted living facilities as required. Collaborates with the Home Care, Palliative and Hospice clinical teams to ensure patients are receiving all beneficial services.
Conducts informational conversations with patients and families about palliative and hospice services. Time is divided between office and travel throughout our service area. Bring your art of relationship building, strong communication skills, and knowledge of patient care to meet the ever-evolving healthcare needs of patients in the community we serve.
This position requires travel within our service area. The right candidate for this role will live within a reasonable commute to New London, NH.

Requirements for the Clinical Liaison:
- Healthcare experience in Homecare, Hospice and/or Case Management
- Affinity for networking
- Comfortable presenting to individuals, and large groups
- Current NH RN, LPN, MSW license or equivalent healthcare experience required
- Excellent written and oral communication skills required
