Job Title: Patient Care Coordinator II
Duties:
- Partner with Primary Care Providers to implement and monitor patients' care plans.
- Coordinate appointments, follow-ups, and pre-certifications with insurance carriers.
- Manage reminders, alerts, and dashboard tasks to ensure smooth patient care progression.
- Resolve billing errors and escalate issues to the Department Manager as needed.
- Serve as a liaison between patients, physicians, and clinical staff, connecting patients with internal and external resources.
- Provide thoughtful guidance to patients regarding healthcare options.
- Contribute to process improvements aimed at enhancing patient satisfaction.
Skills Required:
- 4+ years of experience in healthcare, customer service, or related fields.
- Strong communication and organizational skills.
Education:
- Associate’s degree required; Bachelor’s degree preferred.
- Knowledge of medical terminology highly desirable.
Pay Range:
- Weekly: $1,300 - $1,600
