Position Overview: The Case Manager will be responsible for assessing client needs, developing and implementing individualized care plans, and coordinating services to help clients achieve their goals. This role involves regular client interactions, collaboration with service providers, and ongoing evaluation of the effectiveness of care plans.

Responsibilities:

  • Initial Assessment: Perform a thorough evaluation of the client's physical, emotional, and social needs during the initial meeting. This may include gathering personal history, current circumstances, needs and specific challenges the client faces.
  • Risk Assessment: Use standardized tools and methods to evaluate the client’s risk factors, including any immediate needs for intervention or potential for crisis situations.
  • Strengths and Resources: Identify the client's strengths, skills, and available resources that can be leveraged to support their goals and address their challenges.
  • Collaborative Goal Setting: Work with the client to set realistic, measurable, and achievable goals based on the assessment findings. Ensure that these goals align with the client's needs and desires.
  • Action Plan: Create a detailed action plan outlining the steps required to achieve the established goals. This includes specifying tasks, timelines, and responsible parties for each action item.
  • Resource Identification: Identify and coordinate necessary resources and services including employment, education, healthcare, family reunification, spiritual guidance and housing that will assist the client in achieving their goals and reducing the likelihood of recidivism. This might involve connecting them with community resources, healthcare providers, or support services.
  • Utilize Risk Assessments: Evaluate clients' risk of recidivism using comprehensive risk assessment tools to determine their specific needs and potential challenges.
  • Determine Meeting Frequency: Based on the assessed risk level, establish an appropriate meeting schedule to provide regular, one-on-one support.
  • Regular One-on-One Meetings: Conduct scheduled meetings with clients to address their needs, monitor progress, and implement necessary interventions. Adjust the frequency of meetings as needed to ensure clients are stabilized and progressing towards their goals.
  • Ongoing Evaluation: Continuously reassess clients' risk levels and adjust meeting frequency and support strategies to ensure effective stabilization and successful outcomes.
  • Identify and connect clients with appropriate resources and services, such as medical care, counseling, housing, or financial assistance.
  • Coordinate with healthcare providers, social services, and community organizations to ensure clients receive comprehensive support.
  • Monitor and manage service delivery to ensure it meets clients' needs.
  • Provide regular follow-up with clients to assess progress and adjust care plans as needed.
  • Offer guidance and support to clients to help them navigate challenges and overcome barriers.
  • Maintain up-to-date records of client interactions, progress, and outcomes.
  • Provide supportive counseling, advocacy, and referrals to community resources as needed.
  • Advocate on behalf of clients to ensure they receive the services and support they need.
  • Act as a liaison between clients and service providers to address any issues or concerns.
  • Educate clients about their rights and available resources.
  • Maintain accurate, timely and confidential records of client information, interactions, assessments, progress notes and care plans.
  • Prepare reports and documentation for internal use, regulatory compliance, and funding purposes.
  • Ensure that all documentation adheres to legal and ethical standards.
  • Adhere to all confidentiality and ethical standards in accordance with organizational policies and legal requirements.
  • Work closely with multidisciplinary teams, including parole and probation officers, CPS caseworkers, healthcare providers and community partners to ensure holistic care of clients.
  • Facilitate communication between clients and their support systems to ensure cohesive care.
  • Coordinate quarterly team reviews with client’s support system as required.
  • Respond to and manage crisis situations, providing immediate support and resources as needed.
  • Develop and implement crisis intervention plans when necessary.
  • Stay informed about best practices, regulations, and emerging trends in case management.
  • Pursue continuing education and professional development opportunities to enhance skills and knowledge.
  • Participate in case conferences, team meetings, and professional development activities

Qualifications:

  • Bachelor's degree in Social Work, Psychology, Counseling, or a related field.
  • Proven experience as a Case Manager or internship, preferably in a nonprofit setting.

Skills:

  • Strong interpersonal, assessment and communication skills.
  • Excellent organizational and time-management abilities.
  • Ability to handle complex cases, manage multiple priorities and work independently as well as part of a team.
  • Proficiency in using case management software and other relevant tools.
  • Understanding of community resources, social services, and healthcare systems.
  • Ability to work effectively with individuals from diverse backgrounds and cultures.
  • Compassionate and empathetic approach to client care.
