Hours: Per Visit
Position Summary:
The Per Visit Physical Therapist functions as a key member of the Homecare department through the administration of Physical Therapy services under physician orders in accordance with CMS and DPH Home Health regulations, best practice and quality standards.
Come work with highly skilled interdisciplinary therapy team with specialized training in:
- Advanced Total Joint REplacement Program
- Evidence Based Fall Prevention Programs (Tai Chi/Otago/CT Falls Collaborative)
- Lymphedema Therapy
- PHIT (Pelvic Health and Incontinence Program)
- Modalities (Kinesio Taping/Biofeedback/Ultrasound/TENS)
- CORE: Oncology Rehabilitation
- Rehabilitation: Pulmonary, Cardiac and Vestibular Rehabilitation (including Epley Maneuver/Canalith Repositioning)
- Geriatric Certification
- Pediatric Rehabilitation
- Hospice/Palliative PT/OT/ST
- Cognitive Testing (“MOCA” Montreal Cognitive Assessment)
- Orthotic/Prosthetic Evaluation, Fitting and Training
- Low Vision Assessments and Home Modifications
- Maintenance Care (ALS/MS)
Minimum Qualifications:
- Graduate of an accredited School of Physical Therapy
- Licensed or eligible for licensure to practice physical therapy in the State of Connecticut
Preferred Qualifications:
- Experience with electronic medical charting
- 3-5 years’ clinical experience
- Flexibility with scheduling
