Hybrid Clinical Assessor (Nurse Practitioner)

Location: Hempstead, NY (Nassau County - Hempstead, NY)
Job Type: PRN (Minimum 12 hours per week)

Pay: $90.00 - $100.00/hour | Weekly Pay!

Flexible Hours:

  • Monday-Friday: 8:30 AM - 7:00 PM
  • Saturday-Sunday: 10:00 AM - 6:00 PM

What You’ll Do
  • Conduct Assessments: Perform in-person and Telehealth evaluations for individuals eligible for long-term care, ensuring accuracy and compassion.
  • Mentor Clinicians: Train and guide new team members on clinical best practices and assessment protocols.
  • Regulatory Compliance: Stay updated on industry regulations to maintain high-quality standards.
  • Collaborate & Improve: Work with managers to identify opportunities for enhancing processes and quality of care.
  • Lead Initiatives: Participate in projects, lead workgroups, and contribute to program delivery improvements.
  • Quality Assurance: Engage in activities to ensure program goals and quality measures are met.
What We’re Looking For
  • Education: Master’s degree from an accredited Nurse Practitioner program.
  • Experience:
  • 2+ years of clinical experience as a New York Board Certified Nurse Practitioner.
  • 1+ year of training, precepting, or clinical guidance experience.
  • Preferred backgrounds: Internal Medicine, Adult Health, Community Health, Family Practice, or Psychiatry.
  • Licensure:
  • Active New York State Nurse Practitioner license.
  • NY Medicaid ID Number (or willingness to obtain).
  • Skills: Experience with adult/geriatric populations, home care, or individuals with disabilities is a plus.
  • Travel: Ability to travel up to 60% in the assigned region.

Pay Details: $90.00 to $100.00 per hour
