Case Management is a collaborative practice including patients, caregivers, nurses, social workers, physicians, payers, support staff, other practitioners and the community. The Case Management process facilitates communication and care coordination along a continuum through effective transitional care management and utilization management.Recognizing the patient's right to self-determination, the significance of the social determinants of health and the complexities of care.
Utilize your transferrable skills to assess and coordinate care for a diverse population of patients in the community. Collaborate with our Multidisciplinary Team as well as patients and their families to ensure safe, high quality, and effective delivery of care.Join our Care Management Team at our hospital!

Full-Time Variable shift (Day & Evening shifts).
Experience:Two years: Experience in healthcare related field (Acute, Ambulatory, or Post-Acute)
Two years: Experience in Acute Care Management (Care Coordination) or successful completion of the Transitions in Practice (TIP) program for Care Managers.
Education:Master's Degree: Social Work from an accredited program.
Certifications|Licensures:Upon hire: California Registered Nurse License
Ideal Candidate: Master's Degree - Social Work from an accredited program.
California Clinical Social Worker License upon hire.2 years - Experience in healthcare related field (Acute, Ambulatory, or Post-Acute).
2 years - Experience in Acute Care Management (Care Coordination)
Special Skills: Must be able to use independent decision-making skills in a wide variety of situations
Frequent contact with employees, physicians, customers, community individuals and volunteersMust be self-motivated to complete work in a timely manner to meet deadlines
Experience with Core Measures, Quality Indicators, Case Management, and Utilization
