Job Title: Patient Navigator - Care Coordination

Duties:

  • Guide patients through the care process by coordinating appointments and ensuring timely follow-ups.
  • Support patients with information on treatment plans and available resources.
  • Utilize Epic for patient records and scheduling, maintaining accuracy and confidentiality.
  • Act as a liaison between patients and healthcare providers, fostering strong communication.
  • Provide administrative support, including data entry and report generation using Excel.

Skills Required:

  • Experience in a patient-facing role within healthcare.
  • Familiarity with Epic and strong communication skills.

Education:

  • Associate degree required.

Pay Range:

  • Weekly: $1,300 - $1,550
