Job Description:

  • As the MDS Coordinator, you will oversee the completion and management of the Minimum Data Set (MDS) assessments, ensuring the accurate evaluation of residents' health and well-being in Medicare/Medicaid certified beds. You will collaborate with interdisciplinary teams, ensuring compliance with state and federal regulations while contributing to the overall care planning process. In addition to MDS responsibilities, you may provide direct patient care, assist with staffing needs, and manage administrative duties delegated by the Director of Nursing (DON).

Responsibilities:

  • Complete MDS assessments upon admission, quarterly, annually, and as required by skilled services (PDPM) schedules.
  • Review medical records and care plans for accuracy and compliance with state and federal standards.
  • Coordinate the development and implementation of individualized nursing care plans, addressing the physical, psychological, social, rehabilitative, and discharge planning needs of residents.
  • Supervise nursing care and ensure the proper delivery of medications, treatments, and interventions according to the plan of care.
  • Handle scheduling, accuracy, validation, and timeliness of MDS processes.
  • Communicate with medical records, billing, and appropriate personnel regarding insurance and coding issues.
  • Maintain the resident roster, census, and ensure accurate documentation.
  • Assume administrative responsibilities, including disciplinary actions, as delegated by the DON.
  • Ensure compliance with facility policies, OSHA guidelines, CLIA regulations, and other relevant regulatory agencies.
  • Train new employees, students, and float staff in MDS processes and other nursing practices as needed.
  • Educate residents, families, and staff on care modalities, medications, and discharge planning.
  • Manage and supervise patient care as needed during urgent staffing shortages.
  • Provide specialty care, symptom management, and support for high-risk patients.
  • Be available for emergencies and coordinate with other healthcare providers to ensure continuity of care.

Requirements:

Licensure & Certifications:

  • MI State RN License
  • BLS - AHA Certification

Experience:

  • Minimum of 2 years of Charge Nurse/Supervisor experience.
  • Long-term care experience required.

Skills:

  • Proficiency with MDS software, CMS transmission, and Michigan Care Improvement Registry (MCIR).
  • Demonstrates critical thinking, strong assessment skills, and resident evaluation abilities.
  • Competent in electronic health record systems and computer functions such as iQIES.
