Clinical Medical Social Worker - Linden Primary Care Center
The Clinical Medical Social Worker is responsible for providing developmentally appropriate, outcome-focused diagnostic and therapeutic service to patients and families in accordance with the scope of practice as defined in Chapter 4757 of the Ohio Revised Code, Section 4757.01 et seq. The Clinical Social Worker functions as a member of the interdisciplinary team and provides clinical social work services to patients and their families to improve or maintain social, emotional, functional and physical health: provides consultation, education, and training to staff regarding psychosocial issues that impact the delivery of optimal health care services; and participates in the development and delivery of education, research and continuous quality improvement.Demonstrates competencies defined (*) in Clinical Social Worker responsibilities in providing age-appropriate care to patients from the age of birth to 21 years, and some select adult patients.
Schedule: Monday-Friday 8:30am-5:00 pm

Responsibilities:
- Functions as a member of the interdisciplinary team and provides Clinical Medical Social Work services to patients and their families to improve or maintain social, emotional, functional and physical health (Continuously).
- Provides consultation, education, and training to staff regarding psychosocial issues that impact the delivery of optimal health care services (Frequently).
- Participates in the development and delivery of education, research and continuous quality improvement (Frequently).
Qualifications:
Education: MSWLicensure: LSW, LISW preferred
