Job Summary:We are seeking a dedicated Licensed Practical Nurse (LPN) to provide high-quality patient care in a medical/surgical environment. The LPN will work under the supervision of a Registered Nurse or Manager, delivering age-appropriate interventions that address the physical, emotional, developmental, psychosocial, and educational needs of patients.
The position involves coordinating care and serving as a patient educator while adhering to nursing protocols. The LPN may also participate in call rotations and weekend visits as necessary.
Core Responsibilities:Direct Patient Care:
- Assess and manage patient care following physician orders, under RN oversight.
- Perform routine nursing procedures according to patient needs.
- Meet established productivity standards.
- Collaborate with RNs and other healthcare team members to deliver effective patient care.
- Update RNs, therapists, and clinical supervisors on changes in patient conditions.
- Assist in developing and revising individual care plans with RN input.
- Educate patients and caregivers to foster self-care and support.
- Report concerns regarding patient or staff safety to appropriate authorities or leadership.
Staff Education and Professional Development:
- Act as a preceptor for new LPN employees.
- Participate in continuing education and in-service training.
- Maintain current clinical knowledge and stay updated on home health care practices.
Required Minimum Education:
- High School Diploma or equivalent.
- Certificate in Nursing (LPN).
Required Minimum Licenses and Certifications:
- Licensed Practical Nurse (LPN).
- Basic Life Support (BLS).
Preferred Certifications:
- Valid Georgia Driver’s License.
- Private Vehicle Insurance.
Required Minimum Experience:
At least 1 year of medical/surgical nursing practice in an acute care environment is preferred.
